FOR PATIENTS

A referral to CPMA by your doctor is required to become a patient under our care. Once the referral is received it will be reviewed and triaged by our team.

New patients will be seen according to the appropriateness of the referral, roster space, urgency and physician availability.

CPMA physicians see patients in their home or at the local hospice.

​

A CPMA team member will contact you when there is space available for an initial visit to assess your health needs and make suggestions for improved symptom control.
